a registered Social Worker with 16 years of dedicated experience in Suffolk's adult social care landscape. I have created Suffolk Outreach Pathways to offer the full range of my professional skills and knowledge directly to you.
Offering you complete flexibility to meet your unique needs: a one-off session for immediate help, regular ongoing visits for consistent support, or longer-term arrangements where required.Assisting with tasks that make your daily life easier, more manageable, and enjoyable; help in your home, garden, or out and about. I'm good at listening, providing advice, offering companionship and gentle encouragement. I provide person centred, trauma informed support, with empathy and practical insight.For more targeted needs, I can provide structured outreach services that support your recovery, resilience and growth. Flexible Support Options & Funding

Many people qualify for outreach support funding following a Care Act assessment of their needs with the local authority . This can be arranged with a Direct Payment budget tailored to your situation — often with no or low cost to the individual, depending on your financial assessment.This includes one-off sessions or short-term packages (up to 6 weeks) to help you live independently at home after a crisis, hospital discharge, or significant life change.How It Works
Through a Care Act assessment or professional referral, the Council may offer a Direct Payment budget tailored to your needs. These enable person-centred support that empowers you to rebuild routines, manage challenges, and achieve sustainable independence.

Service Area & Travel

Based in West Suffolk between Bury St Edmunds and Mildenhall, I primarily cover a 25-mile radius across Suffolk, Norfolk and Cambridgeshire. This includes Mildenhall, Brandon, Newmarket, Thetford, Ely, Stowmarket, Haverhill, Sudbury and parts of Cambridge for quick, reliable responses.I am happy to travel further across Suffolk, Norfolk and Cambridgeshire for significant or longer-term packages where practical. Mileage is charged at the standard 45p per mile (often reclaimable from council funding).
